ACW North Smithfield, RI 1105 Iron Mine Hill Rd November 25, 2009 : North Smithfield, RI - Companies responded to 1105 Iron Mine Hill Road for a reported commercial garage fire around 0110hrs on November 25, 2009. Engine 2 arrived first due and reported smoke showing from a 1 story wood framed storage garage. The crew stretched in a 1.75" hand line through side 1 and found heavy smoke inside but no visible fire. Tanker 1, working it's first fire,  arrived and set up water supply to Engine 2 via a 2.5" line. Smithfield Engine 2 was special called on the first alarm response. Smithfield was released to go cover North Smithfield Station 2 due to the quick knock down. The fire was contained to the area of origin near a wood stove.

3rd Alarm Worcester, MA Vinson Street November 24, 2009 : Around 1745hrs on November 24, 2009 the Worcester Fire Alarm started receiving calls for a building fire in the area of 103 Rodney Street. Box 16-09 was transmitted soon after for several calls being received. Engine 16 , who was on the air, reported heavy smoke showing. The first due companies reported heavy fire from the side 2-3 corner from the first floor to the third overlapping the roof line of a 3sty wood frame occupied dwelling. The companies reported to fire alarm that the address was actually on Vinson Street just up from Rodney Street. Command arrived and requested a second alarm on his arrival. Engine 16’s firefighters hand jacked their attack lines up Vinson from Rodney to attack the fire. The dwelling sat up on a steep grade which made hand line placement difficult. The front of the dwelling was left open for three ladder truck to open up and protect exposures if needed. A third alarm was struck for relief purposes due to the size of the structure and the grade of the roadway. The heavy fire was knocked down in 30 minutes. Companies remained on scene for overhaul of the three floors and attic area for close to 3 hours. The cause is under investigation and an occupant was treated for burn injuries by Worcester EMS. A total of 8 engines, 5 ladders, 1 heavy rescue, EMS units, & several chief officers  worked the scene of this third alarm of fire.

MVA Burrillville, RI Rte-102 (Broncos Hwy) & Central Street November 20, 2009 : Burrillville, RI – Fire Dispatch toned out Stations 1 and 3 for a reported motor vehicle accident in the area of Rte-102 and Central Street around 2350hrs on November 20, 2009. Engine 13, Medic 1, Rescue 3 all called enroute within minutes and dispatch replied with, “We are now receiving several 911 calls regarding this crash”. Engine 13 radioed that they had a two vehicle head on accident with entrapment as they arrived on scene. Station 2 was requested for a second set of jaws due to the drivers in both vehicles being heavily pinned. Firefighter/EMTs climbed into the twisted vehicles to assess the patients and hold cervical spine stabilization. Firefighters readied their hydraulic tools and gained purchase points to being the task of freeing the drivers. They had to use cutters, spreaders, and battery operated reciprocating and circular saws to remove the warped metal.  A hose line was stretched from Engine 21 to protect the scene if a fire was sparked. EMTs initiated IV access inside the vehicles while the extrication continued. The OIC requested a third set of jaws from Station 4 and also for a Lifeflight helicopter to the scene. The driver of the silver sedan was removed in 35 minutes and the black sedan’s driver was removed in 45 minutes. The flight crew was transported to the scene from the landing zone to triage their patient.The silver sedan’s driver was transported ALS to Rhode Island Trauma Center by Medic 1. Rescue 3 brought the flight crew and patient to the landing zone at the middle school. The patient was flown to the trauma center of the University of Massachusetts.

ACW Woonsocket, RI 46 Newton St November 20, 2009 : Woonsocket, RI - Squad 4, Engine 2, Engine 3, Rescue 1, Ladder 2 and Chief 1 responded to the area of 46 Newton Street for a reported structure fire. Squad 4 arrived to find fire in a 2 bay detached garage with 2 vehilces stored inside. Attack lines were placed into operation on heavy fire on side 3 that had extended into the attic space of the garage. Two lines were stretched and operating on the fire. After a quick knock down, overhaul operations began on the underside of the roof rafters and the side 3 outside and inside wall.
